Requesting Supervisor confirmation of the Learning Contract

  Once you have agreed to the three learning contract proficiencies by self assessing, you will need to send them to your Assessor, Educator/Manager and/or Clinical Centre Manager for confirmation (who can confirm each proficiency).

  
  

  1. Select Proficiencies confirmation.

  

  
  

  
  

  2. Select New confirmation request.

  
  

  3. Select which Supervisor you would like to confirm the proficiency and select Next

  
  

  If you cannot see the supervisor you want, you may need to add them to your supervisor list.

  
  

  
  

  

  
  

  
  

  4. Select the proficiency/proficiencies you would like to send to your selected person and select Next

  

  
  

  
  

  5. Review and select Submit

  
  

  
  

  6. The Confirmation status next to these proficiencies will update to Awaiting confirmation

  
  

  
  

  7. If you want to check the details of your confirmation request, you can go back into Proficiencies confirmation (at top of page), where you can see:

  
  

  • Educator/Manager or Assessor sent to
  • Date you sent it
  • Option to Resend (send an email reminder) or Withdraw (if you want to send it to a different Educator/Manager or Assessor)

  You will not see the option to Resend for one hour after you have sent.

  
  

  
  

  

  
  

  
  

  8. Once your Educator/Manager or Assessor has confirmed your Learning contract proficiency:

  • The Confirmation status next to these proficiencies will update to Confirmed
  • You will receive a confirmation email from noreply@dls.nhs.uk

  

  
  

  

  
  

  
  

  9. Repeat steps to get confirmation for the rest of the Learning contract proficiencies
